A REFERRAL FOR IN-HOME SERVICES CAN BE MADE BY CALLING BARREN RIVER AGING SERVICES (782-9223) OR (1-800-395-7654). ONCE A REFERRAL IS RECEIVED, A CASE MANAGER WILL CALL THE PERSON NEEDING SERVICES AND COMPLETE A PRIORITY RATING. PERSONS ARE PRIORITIZED BY GREATEST NEED. THAT PERSON WILL GO ON A WAITING LIST.
When someone applies to receive services, a CASE MANAGER makes an “assessment”. An assessment is the collection of in-depth information about a person's problems in the major functional areas. This will identify all the needs and resources so that a comprehensive solution can be planned with the client. Each person who meets the criteria listed in the "Eligibility Requirements" can receive services.
BARREN RIVER AGING SERVICES CASE MANAGERS ASSESS CLIENTS TO DETERMINE ELIGIBILITY FOR IN HOME AND ADULT DAY CARE SERVICES. CASE MANAGERS WORK IN SPECIFIC COUNTIES.
A REFERRAL CAN BE MADE TO THE SENIOR CENTERS THROUGHOUT THE BRADD TEN COUNTIES FOR SERVICES AVAILABLE AT THAT SITE.
